  • An underdeveloped antihelical fold (the inner cartilage fold that gives the ear its curved shape)

When to Consider Otoplasty

The ear reaches approximately 85 percent of its adult size by age 5 to 6 years, making this the earliest appropriate age for otoplasty. Surgical Technique

  • Mustarde sutures: Permanent sutures placed through the cartilage to create or enhance the antihelical fold, bringing the ear closer to the head

  • Cartilage scoring: In cases where the cartilage is particularly stiff, controlled scoring (partial-thickness cuts) on the front surface allows the cartilage to bend into the desired shape

Recovery After Otoplasty

A protective head bandage is worn for one week after surgery. Patients are advised to wear a headband while sleeping for an additional four to six weeks to protect the ears during the healing period. Most children can return to school within one week, and adults can return to work within a few days. Contact sports should be avoided for six weeks. Results are permanent and immediately visible once the bandages are removed.

Microtia Reconstruction

Microtia is a congenital condition in which the external ear fails to develop normally, ranging from a slightly smaller ear to complete absence of the ear (anotia). Microtia affects approximately 1 in 6,000 to 12,000 births and is more common on the right side. It may be associated with narrowing or absence of the ear canal (aural atresia) and hearing impairment on the affected side.

Reconstruction Options

  • Autologous rib cartilage reconstruction: This is the gold standard technique. Cartilage is harvested from the patient's ribs and meticulously carved into a three-dimensional ear framework, which is then implanted under the skin at the ear position. The procedure is typically staged over two to three operations, with the first stage performed around age 8 to 10 years when the rib cartilage is sufficiently developed and the opposite ear has reached near-adult size for comparison
  • Medpor (porous polyethylene) reconstruction: A synthetic framework made from porous polyethylene is used instead of rib cartilage. This can be performed at a younger age (around 5 to 6 years) and often requires fewer stages. However, the synthetic framework carries a slightly higher risk of exposure or infection over the long term

Hearing Considerations
